Do not submit medical, health or other sensitive data through this form.

- Cloudflare hosts the website, provides Turnstile and protects the form against abuse.
- Notion stores requests and notifies the person responsible for following them up.
These services may process the technical data required for their operation according to their own terms and safeguards.
Retention period
A request is retained while it is being handled and for no more than twelve months after it is closed. Closed records that have reached the end of their retention period are deleted monthly.
Your rights
You may request access to, correction of or deletion of your data by emailing the address above. Reasonable identity verification may be required before the request is processed.
